Roadmap For Implementation Of An EHR System At A Practice

From an Information Technology point of view, there are six stages in the implementation road map.

Stage 1:Assessment
Stage 2:Planning
Stage 3:Selection
Stage 4:Implementation
Stage 5:Evaluation
Stage 6:Improvement

Lets examine each stage and develop the frame work of a road map.

Stage 1:Assessment

A Project team represented by staff with the appropriate skills is formed and regular team meetings are conducted. These team meetings continue through out the the six stages.

The first stage Assessment helps prepare the practice for the implementation by completing a Practice Readiness Assessment, This includes The profile of the Practice in terms of IT, assessment of Office staff usually conducted as an assessment Survey. A Hardware requirement analysis is also carried out at this stage."

Stage 2:Planning

The practice data collected in the previous stage (Practice Readiness Assessment, Practice profile (IT)& Office staff skills survey)is carefully reviewed and based on this, the electronic health records implementation goals are defined and improvement opportunities are identified and targeted.

Stage 3:Selection

Based on the above defined Implementation plan, the EHR system's requirements is defined. This will cover the EHR system configuration and a selection process and details of the goals that are archived based on the selection. The EHR system is selected in this stage.

Stage 4:Implementation

An EHR system implementation plan is created and the time lines for the implentation is agreed upon with the vendor. The implementation plan includes details on Intallation and configuration of Hardware and EHR system software. Staff training program is initiated and System testing follows.The staff begin to use the EHR system. A journal of experience and processes is maintained.

Stage 5:Evaluation

A post implementation review is condcuted and the Journal of experience/ processes is updated. The perfomance measures created during the planning phase are validated and a improvement plan is prepared.

Stage 6:Improvement

During this phase the EHR is modified to resolve issues encountered during the evaluation phase. Improvements as defined in the improvement plan are carried out.

Achieving Goals Through Management Systems

Many leaders understand the importance of achieving goals, but have little skill at motivating their subordinates to meet these goals. Often leaders in this position end up blaming the people they lead. Most often the fault lies with the leaders in ability to focus effort toward a specific result.

A thriving leader finds ways to focus effort toward their desired results. For example, the factory manager knows that they want to lower the number of accidents, but having fewer accidents is a fairly abstract concept. However the number of days since the last accident is a very concrete idea that is easy for everyone to grasp. It helps measure the concept of having fewer accidents in a way that is easy to comprehend and measurable. Many factories have found they can lower the number of accidents simply by making people cognizant of how well they are doing toward achieving the goal.

The difficulty is to find proper thing to measure. There was an IT department where the manager decided to measure the number of trouble tickets they closed each week. This metric was used as part of the employees? performance review. However, if everything was running just fine with no problems, there were no trouble tickets to close.

The employees began shutting down networking equipment for 10 minutes at a time, so users would open a bunch of tickets. Then they would turn the equipment back on and close all of the tickets. This increased the metric used for their review.

If an IT department is functioning well, the number of problems from system outages will be very low. The manager basically created problems because problems were what was being measured.

Making the metrics visible keeps people focused on the desired results. A skilled leader can identify the measurements of success and come up with innovative ways to make those metrics visible and easy to understand.

Chronic Fatigue System - How Does it Affect Sleep?

Or to turn it around - How does improving your sleep lessen the symptoms of chronic fatigue syndrome (CFS)?

Our aim here is to understand the connection between the two as it may be possible to confuse the symptoms of CFS and sleep deprivation. There are however definite differences.

Chronic Fatigue Syndrome Symptoms

Constant and persistent fatigue lasting 6 months or more

Feeling feverish

Sore throat

Muscle aches and pains

Tender or swollen lymph nodes

Joint pain

More headaches than usual

Abnormal lethargy after exercise

Poor concentration

Depression

Poor quality sleep

Although not everyone who suffers with CFS will display all of the above, two or more of these symptoms would definitely bear further investigation.

CFS often follows a bout of flu or other respitory disease. It occurs more frequently in middle age and twice as often in women as men.

CFS is a debilitating illness which is hard to diagnose due to the symptoms being non specific. Also the symptoms described by the sufferers often cannot be verified. For instance, patients often describe feeling feverish although their temperature is normal. Recent studies indicate CFS may in fact be an immune disorder.

There are medical practitioners who still believe it to be a disease of "malingerers". In fact some of those suffering with CFS often wonder sometimes if they are imagining their symptoms. But CFS is very real and can lead to broken marriages and wrecked careers.

It is very important for those who suspect they have CFS to seek medical attention straight away. If you feel your doctor is not taking you seriously, get a second and even a third opinion. It helps if you have been seeing the same doctor for some time, as they know you and can recognize that you are not a "hypochondriac" or a time waster looking for sympathy.

CFS is certainly not new, although the association of symptoms under the name Chronic Fatigue Syndrome only took place in the 1990s. Previously it was largely overlooked or thought to be something else, for example low blood sugar, Lyme disease, allergies, fybromyalgia, yeast infections etc.

Chronic Fatigue Syndrome Treatment

These vary from antidepressants, steroids, immune system medications and anti-histamines to the more holistic treatments like evening primrose oil, diet therapy, cognitive behavioural therapy, removal of dental fillings (mercury poisoning) and gentle exercise programs.

Although lack of a definite treatment for CFS leaves a lot to be desired, the above treatments, whether all or a combination, can prove fairly effective. The broad answer seems to be to treat the symptoms as they occur.

Therefore the quality of sleep of patients must have a direct effect on the severity of the disease. If depression is the main problem, treatment of that can significantly improve the quality of sleep, thus relieving somewhat the persistent fatigue.

A combination of better quality sleep with gentle exercise, optimal diet and cognitive behavioural therapy would go a long way to improving the lives of patients with CFS.
